The DES states that Clinical Pharmacists should be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews in people with learning disabilities or autism, through STOMP ( Stop Over Medication Programme) but we don’t always feel we have the clinical expertise to intervene with some of the medicines prescribed or the communication skills to undertake SMRs in this cohort of patients.
Working with CWPT, the Health Innovation Network Medicine Safety programme and CPPE, our delivering STOMP through SMRs workshop is designed to help equip attendees with the knowledge and skills to feel confident to make appropriate interventions and reduce inappropriate prescribing in patient with learning disabilities.
