This training session will support practices in the delivery of the Annual Health Check DES. The training session will be led by the Community Learning Disability Team, Coventry and Warwickshire Partnership Trust and will cover:
- Identification of patients with learning disabilities
- Clinical coding
- Health needs associated with learning disabilities
- Preparing for annual health checks
- What annual health checks should cover
- Reasonable adjustments
- Communication tips
- Health action plans
- Solutions for overcoming barriers
- STOMP Initiative (Stopping over medication of people with a learning disability)
- LeDeR (Learning Disabilities Mortality Review) Programme
This invitation is open to all staff but will be of particular benefit to those involved in the delivery of annual health checks for people on your practice LD register, including the lead GP for learning disability, lead Practice Nurse and Practice Manager/Senior Receptionist.
