We need your help to highlight and address discrimination and racism in general practice across C&W. This will enable us to put support in place; improving the experience of staff in general practice and impacting patient care.
We appreciate you are all extremely busy, this confidential survey should take no more than 10 minutes to complete.
This survey is for everyone working in general practice across C&W, of any ethnicity; we ask that you answer as openly and honestly as you can.
We recognise that answering some of the questions might be triggering and upsetting for some of those with lived experiences of racism or discrimination. There are a number of options available if you do require support or if you would like to talk to someone confidentially:

- Speak to your Health and Wellbeing Champion
