About this event
This event is open to all healthcare professions in primary care and other sectors who are involved in the diagnosis and management of Hypertension which includes GPs, Practice managers, Nurses (community, practice-based, hospital), Health Care Assistants, Pharmacists (community, ICB, practice-based, hospital), social prescribers, health and wellbeing coaches, paramedics, physicians associates etc.
Content
The content of the workshop will be designed to help support primary care teams deliver on the cardiovascular ambitions outlined in the NHS Long Term Plan and meet the requirements of the CVD PCN DES as part of the national CVD Prevent audit.
At the end of the workshop, attendees will be able to:
- Demonstrate an understanding of the priorities outlined in the NHS Long Term Plan and the PCN DES with regards to the detection and management of hypertension and its importance in the context of cardiovascular disease risk modification
- To better understand guideline recommendations (NICE NG136 2019) and how to apply these to the management of people with hypertension
- To better understand the common pit falls of taking a measurement and how to counsel patients how to take their blood pressure
- Confidently diagnose hypertension and be aware of when secondary care referral is recommended
- Analyse clinical and patient information for people with cardiovascular disease and understand how to apply this to their management (pharmacological and non-pharmacological) being mindful of the holistic nature of management.
- Be aware of the projects available locally to help you support your patients
- To help teams align clinical content with opportunities to meet targets as outlined in QOF, the CVD PCN DES and Investment and Impact Fund.
