Support for Primary Care
We are here to help PCNs embed personalised care into their day-to-day interactions with patients.
You will all be familiar with the requirement for all patient facing staff to undertake key training in personalised care – shared decision making by the end of September 2022 and to fulfil the DES requirements for personalised care and support plans for patients.
Shared Decision Making (SDM) – why is it so important?
There are a number of key drivers for shared decision making, which are relevant to anyone looking for levers to improve their services, whether they are healthcare professionals encouraging their team to make improvements or commissioners working for better care across their area.
The NHS Long Term Plan says personalised care will become business as usual across the health and care system, based on ‘what matters’ to people and their individual strengths and needs. Universal personalised care confirms how we will make this happen. It sets out a comprehensive model of personalised care, with shared decision making as one of six key parts of the whole model.

Our Offer:
In the short term, to support this – we are repeating our offer to support PCNs to undertake one of the five modules from the PCI training suite: Your learning options (personalisedcareinstitute.org.uk) by providing a one-off reimbursement payment for each learner.
The reimbursement offer:
The offer is to help with the training requirements for your workforce.
In particular, we are keen to support staff in your PCN with a patient facing role to undertake personalised care training via the PCI website and claim for a one-off reimbursement of their time (equivalent to one hour).
We are asking you to submit an expression of interest for your requirements, but do not envisage being able to provide financial assistance to enable all of your workforce to undertake the training across all of our 19 PCNs, if demand is high, so we will review demand and apportion funds fairly and equitably.
There is a cap on the number of GP’s who can be included in the training – this is capped at 50% of your request – this is to ensure that all roles are considered for personalised care training.
We will consider applications on a first come first serve basis, but will aim where possible, to provide support to all PCNs who make a request.
In consultation with colleagues in the ICS & Training Hub we are offering the following rates:
- GP’s: £100
- Practice Nurse: £25
- Advanced Nurse Practitioner: £30
- Health Care Assistant: £15
However, for ARRs roles where there are no ICS rates, we are offering the following:
- ARRs Clinical: £50
(Clinical pharmacist/pharmacy technician/physio/occupational therapists/dieticians/podiatrist/paramedics/physician associate…there may also be some Mental health workers
- ARRs Non-clinical: £25.00

How to apply:
- Each PCN to submit the attached EOI and return to GEHPersonalisedCare@geh.nhs.uk
- On receipt of an agreed reimbursement – please share the training link with staff
- On completion of course – learners will need to submit a questionnaire to enable payments to be authorised – these can be done individually or collectively from the PCN – returned to: GEHPersonalisedCare@geh.nhs.uk
- Reimbursement will be paid when all agreed staff have completed their course.
In addition – we are keen to pilot a “Prepared Patient” Campaign in one or more PCN. Shared decision making only really works if the patient too is supported to have a different conversation and we are preparing some local resources which we would like to pilot.
